Foxbridge is a licensed assisted living community that provides a variety of healthcare services to support your independence. These services are coordinated by our Medical Director, who is a physician and Geriatrician, and the Wellness Department. You are an integral member of the team and you are encouraged to take an active role in your healthcare.

How are monthly rates determined?

The monthly rate is determined in two ways:

1. The size of apartment you choose. We offer one bedroom, and two styles of studio apartments.

2. The level of care that is appropriate for your needs. Prior to moving to Foxbridge, we will conduct a survey of your health needs. This survey is referred to as a Level of Care Assessment.

What is included in the monthly rate?

Foxbridge offers four levels of care; Basic Level, Level I, Level II and Level III.

The Basic Level monthly rate includes the use of a private apartment for you to decorate with your furnishings, three meals and snacks daily, scheduled transportation, housekeeping with linen and laundry service, recreational programs, emergency call system, monitoring of vital signs, medication supervision, daily reminders and utilities except telephone and cable television service.

If you need more than the Basic Level of Care, we offer many assisted living services. These services may vary according to your needs. Level I and higher may include a combination of the services listed below. Your Level of Care Assessment will determine the right combination and frequency for you.

• Specialized care for dementia
